How Pregnancy Changes Your Body’s Mechanics

From around the first trimester, the hormone Relaxin begins loosening the ligaments throughout your pelvis in preparation for birth. This is entirely normal — but it means the joints that ordinarily provide stability become more mobile and, in some cases, more vulnerable to misalignment.

As the pregnancy progresses, the forward shift of your centre of gravity increases the curve in your lower back, alters your gait, and places compressive load on the lumbar spine and sacroiliac joints. Your upper back simultaneously rounds to counterbalance the weight at the front. These are adaptations, not pathologies — but when the pelvis or spine loses its alignment under that load, discomfort tends to follow.

The Role of the Pelvis in Pregnancy and Birth

Pelvic alignment is central to pregnancy chiropractic for two interconnected reasons: maternal comfort, and the space available to the baby

When the pelvis is out of balance, it can create uneven tension in the uterine ligaments — particularly the round ligaments, which attach the uterus to the pelvic wall. This tension may restrict the space the baby has to move and settle into an optimal position ahead of birth. The clinical term for this restriction is intrauterine constraint.

A well-aligned, freely mobile pelvis supports the baby in finding the best position for descent and delivery, and may reduce the mechanical factors that contribute to prolonged or complicated labour.

The Webster Technique

One of the most researched chiropractic approaches in pregnancy is the Webster Technique, developed by the late Dr Larry Webster, founder of the International Chiropractic Pediatric Association (ICPA).

The technique combines specific sacral analysis with adjustment of the pelvis and soft tissue release of the round ligaments. Its aim is to restore balance to the pelvis and reduce intrauterine tension — not to manually reposition the baby, but to create the conditions in which the baby may move more freely.

A study published in the Journal of Manipulative and Physiological Therapeutics (2002) reported an 82% rate of babies moving to a vertex (head-down) position among a group where the Webster Technique was applied. The ICPA now recommends that women with breech presentations consider Webster-trained chiropractors from around 32 weeks, though many practitioners recommend beginning pelvic care earlier as a preventive measure.

When Is the Right Time to Start?

There is no single “right” trimester. Some women begin care in the first trimester to stay ahead of postural changes; others seek help when pain becomes noticeable in the second or third. Many find the greatest relief in the final weeks, when physical load is at its peak and preparing the pelvis for birth becomes a priority.

Postnatal care is equally valuable and often overlooked. Birth — however it unfolds — places significant force through the pelvis and sacrum. A post-natal chiropractic assessment can support the body’s recovery and address alignment issues that emerge in the weeks of breastfeeding, carrying, and sleep deprivation that follow.

What Does the Research Say About Benefits?

The evidence base for pregnancy chiropractic continues to grow. Studies and clinical reviews have associated regular chiropractic care during pregnancy with:

  • Reduced severity of lower back and pelvic pain
  • Shorter reported duration of labour in some populations
  • Improved pelvic floor function and proprioception
  • Reduced likelihood of certain obstetric interventions in some cases

These are associations, not guarantees — individual outcomes depend on the nature of your presentation, your stage of pregnancy, and your broader birth team. A good chiropractor will give you an honest assessment of what is realistic for your specific situation.
